Community Care

El Sol’s Community Care team provides our clients with case management as well as health, safety, and community information and referral services.  Through case management, we identify significant barriers clients face and create goals that will help them meet their health and human services needs and work toward self-sufficiency.  We provide critical health prevention information in a culturally and linguistically appropriate manner for low-literacy, low-income, uninsured and underinsured residents of the Jupiter area.

Case Management

To truly understand the needs of our clients El Sol has an Intensive Case Management Program.

In 2015, El Sol began a Case Management program in order to provide more individualized care to our workers.  Our Case Management team is bilingual, compassionate, and able to relate to our workers and their families with various needs. They also provide our clients with advocacy, and intervention services that link them with the appropriate social programs within El Sol and community partners in Palm Beach County to improve self-sufficiency. Together the Case Manager and worker develop an individualized goal-oriented “action plan” that is created by utilizing the El Sol Self-sufficiency Matrix tool which is reviewed every six months with the case manager.  Our ultimate goal is to improve the quality of life for our workers, their children, and families.

Tisa's Story

Four shots of insulin a day for her diabetes didn’t stop Tisa from attending Zumba in the Park classes.

“Nora’s enthusiasm and delight when she teaches this class makes you want to do it and go!” Tisa said. “This class is a delight and a blessing in my life – it helps me control my diabetes.”

Zumba in the Park, open to all residents, is a fun way to get healthy. Since she began participating in the classes, Tisa went from four shots to zero. Held once a week at Pine Gardens South/Bruce St. Laurent Park behind the Jupiter Library, Zumba in the Park is provided at no cost to the community through our partnership with Healthier Jupiter and the Town of Jupiter.
